ELIGIBILITY

You may use your Flexible Spending Account (FSA) or Health Savings Account (HSA) toward prescription eyewear at twelvesixtynine. Eligible items include:

• Prescription eyeglasses

• Prescription sunglasses

• Optical lenses (single-vision or progressive)

If you’re unsure whether you have FSA or HSA benefits, your employer or insurance provider can confirm the details of your plan.

HOW TO USE YOUR BENEFITS

At checkout, simply use the payment card connected to your FSA or HSA. If your plan requires additional documentation:

• A digital receipt is included with your shipping confirmation

• A printed receipt is placed inside every package

• In-store purchases can be accompanied by additional documentation upon request

OUT OF NETWORK INSURANCE

Twelvesixtynine is considered an Out-of-Network provider for all major vision insurance plans. While we do not bill insurance directly, many clients receive reimbursement by submitting their receipts, depending on the terms of their plan.

 If your provider requests further information, we are happy to help supply any necessary details.

END-OF-YEAR FSA DEADLINES

Most FSA plans reset at the end of the calendar year. If your benefits expire on December 31, you have until 11:59 p.m. ET on that date to place an order using FSA funds. Because plans vary, we recommend confirming your exact deadline with your provider.
